15. 体检证明或健康承诺书：申请人入境后申领工作证时须提交境内检验检疫机构出具的体检证明，建议入境前采用承诺制，提供健康承诺书（模板见附件2），入境后按照规定进行体检。

Materials Required for Foreign Professional Experts (Category B) to Apply for the
"Notice of Foreigner's Work Permit in China"

7. Highest degree certificate (in PDF format, compressed to within 1M).

15. Physical examination certificate or health commitment letter: When applying for the work permit after entry, the applicant must submit the physical examination certificate issued by the domestic inspection and quarantine agency. It is recommended to adopt the commitment system before entry and provide a health commitment letter (see Attachment 2 for the template). After entry, a physical examination shall be carried out in accordance with the regulations.
